Below you will find a list of fields and valid values used to enter your roster file for import using the Provider Import tool.

When loading your roster using the Provider Import tool, you may see invalid providers that are returned in your roster's results. These will need to be corrected so they are successfully loaded into the platform. If you find an invalid provider is not loading due to a field value being incorrect, refer to the information below to find the accepted valid values. Accepted values in the provider import are also detailed in this PDF guide.

Field and Values

10 digit number that must be “Type 1 - Individual” NPI. “Type 2 - Organizational” NPIs are invalid and will not be accepted.

First Name, Middle, Last Name -

Must be an alpha string with less than or equal to 30 characters.

Provider Type - 

Must be one of the following 2-4 character abbreviations. See Valid Provider Types for a list of current provider types that we credential.

Cred Event -

Must be either "initial" or "recred".

Cred Due Date -

Must be a date that is in the future. The date format must be numerical (no words).

Primary Practice State -

Must be one, two-characters. State abbreviation can found here; only states and the District of Columbia are supported. The following areas are not supported:

• American Samoa - AS
• Guam - GU
• Northern Mariana Islands - MP
• Puerto Rico - PR
• United State Minor Outlying Islands - UM
• Virgin Islands, U.S. - VI 

Practice States -

If the provider practices in more than the Primary Practice State on behalf of the organization, list additional states in this field. Comma separated list of two-character state abbreviation using same abbreviation as Primary Practice State above.

Mailing Address 1 ,2 -

Non-zero length string of alphanumeric characters with special characters; max character length 100.

Mailing City -

Non-zero length string of word characters; max character length 100.

Mailing State -

Must be one, two-characters. State abbreviation found here; only states and the District of Columbia are supported, the following areas are not supported:

• American Samoa - AS
• Guam - GU
• Northern Mariana Islands - MP
• Puerto Rico - PR
• United State Minor Outlying Islands - UM
• Virgin Islands, U.S. - VI

Mailing Zip Code -

Five digits with optional hyphen-separated zip+4 digits, e.g. 12345 or 12345-1234.

Ensure non-numeric (spaces, hyphens, parentheses and any other special characters) are removed; if there is a leading “1”, making the value 11 digits, strip this out before validating; validate that the value is 10 digits

8 or 10 digits

Date of Birth -

Must be later than 01/01/1930; preferred format MM/DD/YYYY; YYYY-MM-DD also accepted.

Gender -

Must be:

Female / Male

F / M

License Number -

Non-zero length string of alphanumeric characters; allow for special characters; max character length 20.

License State -

Must be one, two-character state abbreviation found here; only states and the District of Columbia are supported, the following areas are not supported:

• Puerto Rico - PR
• United State Minor Outlying Islands - UM
• Virgin Islands, U.S. - VI 

Social Security Number -

Must be 9 digits, optionally including hyphens

School -

Non-zero length string of word characters; max character length 100; following special characters are permitted: ( ) & . , ‘ -

Graduation Year -

Must be 4 digits; no earlier than 1950; no later than <= current year + 1.

DEA Number -

Two capital letters followed by 7 digits, .e.g. AB1234567

Board Certified -

Must be:
